The ServiceLink Program provides long-term health services to people of all ages, abilities, and income levels. It provides family caregivers with information and support, as well as access to various Medicare and Medicaid options. As the NHCarePath Full Service Access Partner, ServiceLink provides access to the following programs: Information Referral and Assistance, Aging and Disability Resource Center, Options Counseling, NH Family Caregiver Program, State Health Insurance Assistance Program (SHIP), and Senior Medicare Patrol (SMP).
