The Home Visiting New Hampshire program provides regular home visits by a multidisciplinary team to Medicaid-eligible pregnant women and their families until the child is one year old.

The home visits build on the strengths of families through parenting and child development information and health care support.

The program is funded through the New Hampshire Department of Health and Human Services.
